麻醉诱导时呼气末正压对病态肥胖患者血红蛋白去饱和时间的影响 被引量:4

摘要 目的观察在病态肥胖患者全身麻醉诱导时使用呼气末正压(PEEP)对气管插管时血红蛋白去饱和时间的影响。方法将20例病态肥胖患者(体质量指数>40 kg/m2)分成PEEP组及对照组,各10例。PEEP组在全身麻醉诱导时采用间歇正压通气(IPPV)加PEEP(8cmH2O,1cmH2O≈98Pa);对照组则只使用IPPV。结果 PEEP组的血红蛋白去饱和时间相比于对照组明显延长,差异有统计学意义(<0.05)。缺氧之前动脉血氧分压(PaO2)值PEEP组明显高于对照组,差异有统计学意义(<0.05)。结论在病态肥胖患者全身麻醉诱导时使用PEEP可以增加患者气管插管时的血红蛋白去饱和时间。
