膝关节痛风性关节炎关节镜下处理的疗效分析

目的探讨膝关节痛风性关节炎的关节镜下处理的疗效。方法 2008年12月～2010年5月,该科收治15例经保守治疗效果不明显的膝关节痛风性关节炎患者,采用关节镜下膝关节腔探查和清理术联合术后药物和饮食控制等综合治疗。膝关节功能采用Lysholm评分标准评定。术后给予相应阶段的康复指导。结果 15例患者术后未见切口感染及不愈合等并发症,均获得随访,平均18个月(6～36个月),膝关节功能Lysholm评分:术前为42(35～46)分,术后末次随访为90(85～95)分,手术前、后比较差异有统计学意义(P<0.05)。结论关节镜治疗膝关节痛风性关节炎具有起效快、效果明显、关节功能恢复满意及缓解病情进一步发展的优势,且能处理合并的关节内其他损伤,疗效肯定。

Aim To study the curative effects of arthroscopic treatment of gouty arthritis of knee.Methods A total of 15 patients with gouty arthritis of knee were diagnosed by arthroscopy between September 2008 and May 2010.Then biospy and synovectomy were performed under arthroscope,with an average Lysholm knee score of preoperation and after operation.Rehabilitation instructions were performed according to different stages.Results All the patients were followed up for an average of 18（6 to 36）months,with an average Lysholm knee score of preoperation of 42（35~46）and postoperation of 90（85~95）.The difference between preoperation and postoperation was statistically significant（P0.05）.Conclusion Arthroscopy is the reliable choice for the treatment of gouty arthritis of knee.Advantages of this technique are minimal invasion,simple procedure,less time for operation and hospitalization with early rehabilitation,and all the combined lesion can be treated at the same time.
