胃镜活检病理高级别上皮内瘤变的临床意义

Clinical Significance of Gastroscopic Biopsy in Pathological Diagnosis of Gastric High-grade Intraepithelial Neoplasia

【目的】研究胃黏膜高级别上皮内瘤变在胃癌诊断中的意义。【方法】回顾性分析本院自2006年起上皮内瘤变后胃镜活检诊断为高级别上皮内瘤变33例的临床资料。【结果】术后病理证实为腺癌者29例。占所有患者的87．9%（29／33）。侵犯黏膜及黏膜下层的早期胃癌共12例，占术后诊断胃癌的41．4％，占所有患者的36．4％。【结论】胃镜活检病理证实为高级别上皮内瘤变时87．9％已伴有腺癌，临床上对胃镜活检高级别上皮内瘤变应引起足够重视。

[Objective]To investigate the significance of highgrade intraepithelial neoplasia（HGIEN） of stomach mucosa in the diagnosis of gastric caner. [Methods]The clinical data of 33 HGIEN patients diagnosed by gastroscopic biopsy were analyzed retrospectively since 2006. [Results] Twenty nine cases（87.9%） were pathologically confirmed by operation. Among them, 12 cases were early gastric cancer invading mucosa and submucosa, accounting for 41. 4% the cases confirmed by operation and 36. 4% of all the cases. [Conclusion] About 87.9 % of patients who are pathologically confirmed by gastroscopic biopsy may have been complicated with adenocarcinoma. Gastroscopic biopsy of HGIEN should be paid attention.
