腓肠神经营养血管蒂皮瓣修复足踝部皮肤软组织缺损18例报告

Sural bowel neurotrophic vascular pedicle flap to repair ankle ministry skin and soft tissure defect 18 cases reported

目的探讨应用腓肠神经营养动脉皮瓣修复足踝部皮肤缺损的临床应用价值。方法本组18例,男15例,女3例,均为重物压砸伤或车祸致伤致足跟部及踝部皮肤软组织缺损伴骨、肌腱组织裸露,均采用带血管蒂腓肠神经营养动脉皮瓣修复;效果良好。结果 18例中1例出现血管危象,及时行血管解痉治疗而皮瓣成活,1例皮瓣远端部分坏死,而Ⅱ期行植皮后创面愈合,其余病例转移皮瓣全部成活,创面Ⅰ期修复。结论足跟部及踝部皮肤缺损,采用带血管蒂腓肠神经营养动脉皮瓣修复,手术风险小,且不牺牲肢体主要血管,能有效降低肢体的伤残率。保全肢体,是一种行之有效的治疗方法。

Objective To explore the value of clinical application of sural bowel neurotrophic arterial skin flap in repairing skin parenchyma defect at ankle.Methods All 18 patients,male 15 cases, were injured by the heavy pressure or car accident which led calcaneus skin and ankle skin to soft tissure defection with bone,tendon tissue exposed,all adopt vascular pedicle phil bowel neurotrophic artery of skin flap to repair,and the result is good.Results Among the 18 cases, one occurred vascular crises,timely conducted vessels spasmolysis treatment and the flap survived,1 case appeared part necrosis at distal skin flap and healed after doing the graft treatment in second period,the rest of the cases transferred the flaps and survived,the wound recovered in the first period. Conclusion Skin defect at heels and ankles,using pedicle sural bowel neurotrophic arterial skin flaps,surgical risk small and no sacrifice of major vascular,and effectively lower the body＇s disability rate.Body preserve is an effective treating method.
