超声检测肝门部淋巴结的临床意义分析

目的分析超声检测肝门部淋巴结肿大的临床意义。方法对本院2006年至2009年69例患者超声检测的151个肿大淋巴结的超声特征进行回顾性分析。结果病理证实和随访证实的151个肿大淋巴结中,转移性淋巴结76个,35个淋巴瘤,40个炎性淋巴结,长短轴比值(L/S)、髓质显示、个数及回声在良恶性之间比较差异有统计学意义(P<0.05)。结论在肝门部肿大淋巴结的良恶性鉴别诊断中,尚无一项特异性指标,但当L/S<2、髓质变形、回声极低且多发时,应考虑恶性可能性大,积极查找病因。

Objective To analyze ultrasonic detection of hilar lymph nodes of clinical significance.Methods 69 patients in our hospital from 2006 ～ 2009,151 patients with ultrasound ultrasound features of lymph nodes were analyzed retrospectively.Results Pathology and follow-up confirmed that the 151 lymph nodes,76 metastatic lymph node,35 lymphoma,nflammatory lymph nodes40,short axis ratio（L/S）,medulla shows the number and echo the difference between benign and malignant significant（P 0.05）.Conclusion The swollen hilar large lymph nodes in the differential diagnosis of benign and malignant,there is no one specific indicators,but when the L/S 2,medulla distortion,echo,and multiple very low,they should consider the possibility of malignant large,actively look for the cause.
