MRI动态增强扫描对垂体微腺瘤的诊断价值

The diagnostic value of enhanced dynamic scanning of MRI in pituitary microadenoma

目的评价MRI动态增强扫描对垂体微腺瘤的诊断价值。方法回顾性分析临床有头痛、闭经、泌乳、生长发育迟缓等症状的34例经临床及实验室检查证实的垂体微腺瘤的MRI影像资料,分析垂体微腺瘤动态增强前后的MRI表现,并绘出时间——信号强度曲线图。结果 34例垂体微腺瘤中,平扫检出病灶24例,而动态增强扫描均发现病灶,表现为动态早期稍低信号影,似"充盈缺损"。MRI平扫的检出率为70.6%,MRI动态增强扫描的检出率为100%,行McNemar x^2检验,P<0.05,说明两种扫描方法在垂体微腺瘤检出率方面的差别有统计学意义。结论 MRI动态增强扫描能明显提高垂体微腺瘤诊断的可靠性及检出率,降低MRI图像的不确定性的出现率。临床怀疑垂体微腺瘤者,应常规行MRI动态增强扫描检查。

Objective To evaluate dynamic contrast enhanced MRI scan in the diagnosis of pituitary microadenoma value. Results 34 cases of pituitary microadenoma, scan detected lesions in 24 cases, and dynamic enhanced MRI lesions were found, showing slightly lower signal for the dynamic of early movies, like ＂ filling defect. ＂ MRI scan of the detection rate was 70. 6% , MRI dynamic contrast - enhanced scan was 100% , line McNemar X2 test, P 〈 0. 05, Shows two scanning methods in the detection rate of pituitary microadenoma difference was statistically significant. Conclusions MRI can improve the dynamic enhanced MRI diagnosis of pituitary microadenoma detection rate of the reliability and reduce the rate of MRI images appear ambiguous. Clinical suspicion of pituitary microadenomas who should be routinely performed dynamic contrastenhanced MRI scan.
