辛伐他汀对高血压病伴胰岛素抵抗患者血小板活化功能的影响

Effects of Simvastatin on Platelet Activation Function in Patients with Primary Hypertension with Insulin Resistance

目的:探讨辛伐他汀对高血压病伴胰岛素抵抗患者血小板活化功能的影响。方法:30例高血压病伴胰岛素抵抗患者(治疗组)调脂治疗(辛伐他汀20mg/d,治疗4～8周)前后及30例正常对照组的血小板活化功能,测定CD62p(cluster of differentiation,CD62p)、血糖及胰岛素水平,并计算胰岛素敏感性指数(ISI)。结果:①治疗组及对照组治疗前的CD62p分别为(49.21±20.62)%,(2.45±0.53)%,治疗组经辛伐他汀治疗至4周后CD62p降至(21.28±16.11)%,较治疗前明显下降(P<0.01);治疗8周CD62p降至(20.29±14.98)%,但与4周比较差异无显著性意义(P>0.05)。②治疗组及对照组的空腹血糖(SG)分别为(4.51±0.43)mmol/L、(4.16±0.47)mmol/L,空腹胰岛素(SIN)分别为(18.24±11.50)mmol/L、(8.02±1.07)mmol/L(P<0.01),胰岛素敏感指数(ISI)分别为(1.41±0.32)、(1.58±0.26)(P<0.01)。治疗组经辛伐他汀治疗4周后SIN降至(14.23±7.49)mmol/L,较治疗前明显下降(P<0.01);ISI升至(1.56±0.41),较治疗前明显增高(P<0.05)。结论:辛伐他汀可抑制血小板的活性,改善胰岛素抵抗,利于高血压病伴胰岛素抵抗者高血压本身的进展,防止血栓性疾病的发生。

Objective To study the effects of simvastatin on platelet activation function in patients with primary hypertension with insulin resistance.Methods Cluster of differentiation（CD62p）,Fasting glucose,insulin resistance and insulin sensitivity index（ISI） was detected in 30 healthy subjects（control group） and 30 patients with primary hypertension with insulin resistance before and after 4-8-week treatment with simvastatin（20mg/d）（treatment group）.Platelet activation function was compared between two groups.Results ①CD62p in patients with primary hypertension with insulin resistance was obviously higher（P 0.01） comparing with normal subjects after simvastatin treatment.After 4-week simvastatin treatment,CD62p decreased（P 0.01）,but no further change after 8-week treatment（P 0.05）.②Fasting serum insulin was higher（P 0.01） and insulin sensitivity index was lower（P 0.05） comparing with normal subjects.After 4-week simvastatin treatment,fasting serum insulin was decreased（P 0.01）,insulin sensitivity index increased（P 0.05）.Conclusion The results suggest that simvastatin may inhibit the platelet activity of the primary hypertension with insulin resistance,improve the insulin resistance.
