纤维乳管镜在乳头溢液疾病诊治中的应用价值(附72例报告) 被引量:1

摘要 目的:通过纤维乳管镜检查,直接反映病变情况,同时对疾病进行诊断和治疗。方法:对2006年6月-2007年2月72例病人进行回顾性分析。全组病人术前均行乳腺导管鏡、乳头溢液细胞学检查、彩超检查。发现并确定病灶位置,记录病变导管的形态特征,对可疑病例定位切除送病解确诊。结果:溢液涂片检查找到癌细胞和可疑癌细胞6例(6/72),与术后病理相符。B超检查提示,导管内肿瘤16例(16/72),导管扩张症17例(17/72)。乳腺导管鏡提示导管内肿瘤12例(12/72)。其余21例未发现异常。乳管镜检对乳腺癌诊断的灵敏度为93.4%,特异度为98.5%,远高于乳腺导管造影。乳管镜检查结果与病理检查结果诊断符合率97.5%。结论:乳管镜有助于观察乳管内病变情况,从而有效确定乳头溢液疾病的病因及病变部位,诊断病变性质,提高乳腺癌的早期发现率。 Objective To study the application value of fiberoptic ductoscopy system(FDS) in the diagnosis of breast disease.Methods FDS,nipple discharge cytological examination and color B-mode ultrasonic diagnostic equipment examination were all applied before operation for breast disease patients in the experiment.The location of lesion was determined,the morphologic characteristics of impaired mammary glands were recorded,the equivocal case was operated in its localization and the tissues were detected for Pathology.The 72 patients during June 2006 to February 2007 were retrospectively analyzed.Results The cancer cell and the suspicious cancer cell 6 cases were found by nipple discharge cytological smear examination(6/72),in line with the postoperative pathology detection.The B ultrasonic diagnostic inspection showed that ductal tumor 12 cases(12/72),ductal ectasia 17 cases(17/72),the rest were not abnormal.The sensitivity of FDS for breast cancer diagnosis is 93.4%,the specificity is 98.5%,is far higher than the mammary gland drive pipe radiography.The diagnose accordance rate of FDS inspection results and pathology inspection results is 97.5%.Conclusion FDS can contribute to observe the pathological changes in galactophore,determined the cause of nipple discharge diseases,decide the diseased region,diagnose its character,so as to increase the detection rate of prophase breast cancer.
