急性心肌梗死患者心率震荡的特征及临床意义

Clinical manifestation and characteristic of heart rate turbulence in Patients with acute myocardial infarction

目的探讨急性心肌梗死(AMI)患者心率震荡(HRT)现象的特征及临床意义。方法测量和比较45例急性心肌梗死患者(AMI组)及35例健康者(对照组)窦性心率震荡初始(TO)及震荡斜率(TS)。结果 AMI组患者震荡初始(TO)较对照组增加(P<0.05),震荡斜率(TS)减低(P<0.001),TO及TS异常发生率高于对照组。TO及TS无相关性。结论心率震荡可能有预测心肌梗死患者发生恶性心律失常及心脏性猝死的作用。

Objective To study the characteristic and clinical manifestation in patients with ischemic acute myocardial infarction according to heart rate turbulence（HRT）.Methods The study population included 80 patients：group-C of 35 controls,group-M of 45 patients with acute myocardial infarction.Heart rate turbulence was measured on the Holter monitor.Results Impaired HRT was observed most frequently in group-M（P〈0.05）,increase of the turbulence onset（TO）and the decrease of the turbulence slope（TS）（P〈0.001）.In group-M,the accidence of cardiac events were no significant difference between abnormal HRT subgroup and normal HRT subgroup.Conclusion Weaken HRT,increased TO and decreased TS are observed in AMI patients.HRT may be strong and independent predictor of cardiac and arrhythmic death in myocardial infarction patients.
