对山东省基本药物制度及配套制度实施过程中政府投入政策的思考

Study on Financial Input of Government Related to the Essential Medicine Policy and Other Supporting Systems in Shandong Province

基本药物制度是一项全新、系统的制度,建立国家基本药物制度和健全基层医疗卫生服务体系,是未来几年我国深化医药卫生体制改革的重要任务。对于规范药物生产、流通秩序,降低药品费用在医疗费用中的比例,提高居民的健康水平,保证人民群众基本就医需求提供了重要的保障。当前,影响基本药物制度运行的主要障碍就是资金投入问题,即针对政府主导的多渠道补偿机制的探索。目前,山东省针对基本药物零差率补偿、绩效工资改革、医疗保障制度的调整等相关制度已开始全面启动,基层医疗机构对政府财政补助的依赖性极大提高,然而基层财政补偿不足额、不稳定的短期效应也已开始逐步显现,并将直接影响到基本药物制度的进一步扩大。探索如何建立一种科学、持久、充足的政府投入机制,将一直是我们长期关注的重点。

National essential medicine policy is a new and systemic system.Establishing this system and strengthening the development of primary health care institutions are very important tasks in deepening medical and health system reform in the near future.It is the magnitude guarantee in standardizing drug production and circulation,reducing the drug expenses in medical treatment costs,and improving the proportion of residents＇ health level and ensuring people basic medical treatment demand.One of the main obstacles is financial input,which mainly focuses on the leading factor of government of multi-channel compensation.It owns great significance for ensuring the implementation of government policies,reforming the operating mechanism of primary care health facilities,establishing the unity of fairness and efficiency incentives,and improving the work enthusiasm of grass-root medical staffs,etc.Maintaining a scientific,long-lastiong and enough government input mechanism will be our focus point for a long time.
