摘要
腔镜技术在甲状腺癌的应用有增多趋势,腔镜下不但能够完成甲状腺腺叶切除,还可以完成甲状腺癌中央区淋巴结清扫以及侧颈部淋巴结清扫,从而在根治切除甲状腺癌的同时能够最大限度地保护病人的颈部外观和功能。但腔镜甲状腺癌手术的适应证、彻底性以及无瘤原则等方面尚有争议。机器人辅助腔镜手术系统的应用扩大了腔镜手术在甲状腺癌的适用范围。合理选择甲状腺癌腔镜手术的适应证和手术范围,强调综合治疗,避免过度治疗,有利于提高病人的生活质量。
The application of endoscope thyroid surgery including thyroidectomy and neck lymph node dissection increases in thyroid cancer today, which could radically remove the disease at most extent and preserve the neck appearance in the meanwhile. There are controversies on indications, effectiveness and no-touch principle of endoscope surgery in thyroid cancer. The robot-assisted system enlarged the applicable range of endoscope surgery in thyroid cancer. It is necessary to select the suitable indications and operation range and to prevent exeessus treatment.
