超声造影在胆道恶性肿瘤诊断中的应用研究

Application of Contrast-enhanced Ultrasound in Diagnosis for Malignant Tumor of Biliary Tract

目的:探讨超声造影在胆道恶性肿瘤诊断中的应用价值。方法:对25例经过二维超声及其他影像学方法检查后诊断为胆道恶性肿瘤患者均行超声造影检查,观察病灶开始增强时间、达峰时间及持续时间,并比较病灶增强形态。结果:造影后25例胆道肿瘤病灶均可见不同程度增强,其中胆囊癌17例,胆道癌8例。各种瘤病灶均较二维超声显示更清楚。二维超声误诊的2例胆囊癌及2例胆总管癌均经造影后增强而得到确诊。结论:胆道恶性肿瘤超声造影回声增强具有特征性,有助于提高胆道恶性病变的检出,超声造影对胆道疾病的诊断及鉴别诊断有较好的应用前景。

Objective： To explore the value of contrast - enhanced ultrasound （ CEUS ） in the diagnosis for malignant tumor of biliary tract. Methods ： 25 case with malignant tumor of biliary tract underwent CEUS examination after conventional ultrasound or other medical imageology . The time of initial enhancement , peak enhancement , and initial decrease were observed , and enhancement appearances of lesion were compared . Results： After injection of SonoVue , all tumor of biliary tract in 25 cases shoeed enhancement and these lesions looked clearer than those on conventional ultrasound. Among them, 17 cases were primary gallbladder carcinoma , 8 cases were cholangiocarcinoma . 2 cases gallbladder carcinoma and 2 cases cholangiocarcinoma who were misdiagnosed on conventional ultrasound were diagnosed on CEUS with enhanced tumor lesions . Conclusions： CEUS can provide on important supplement to diagnosis and may further improve the diagnostic accuracy for malignant tumor of biliary tract.
