慢性心力衰竭中西医结合生存质量量表与SF-36简明健康状况量表信度、反应度对比研究

Study on the Reliability and Responsibility of Chronic Heart Failure Quality of Life Scale of Integrated Chinese and Western Medicine with the 36-item Short-form Health Status Survey

目的以SF-36简明健康状况量表(SF-36量表)为标准,评价慢性心力衰竭(慢性心衰)中西医结合生存质量量表在临床实际运用中的信度、可行性及反应度。方法采用横断面调查法,对符合纳入标准的63例慢性心衰住院患者进行生存质量的调查,内容包括慢性心衰中西医结合生存质量量表和SF-36量表。采用克朗巴赫α系数、分半信度评价两个量表的信度,根据完成量表时间、完成率评价量表的可行性,通过测评慢性心衰患者治疗前后总体生存质量评分的变化来判别量表的反应度。结果 SF-36量表总的克朗巴赫α系数为0.896,总分半信度系数为0.903;慢性心衰中西医结合生存质量量表总的克朗巴赫α系数为0.951,总分半信度系数为0.960。SF-36量表平均应答时间为(10.9±4.6)min,慢性心衰中西医结合生存质量量表平均应答时间为(12.0±4.9)min,两者的完成率均为96.83%。两种量表治疗前后生存质量总分差异均具有统计学意义(P<0.01)。结论与标准参照量表SF-36量表相比,慢性心衰中西医结合生存质量量表具有较好的信度和反应度,临床操作可行,可以作为心力衰竭中西医结合生存质量的评价工具。

Objective Taking the 36-item short-form health status survey （SF-36） as a standard,to evaluate the reliability,feasibility,and responsibility of the Chronic Heart Failure Quality of Life Scale of Integrated Chinese and Western Medicine. Methods A cross-section survey was conducted. Sixty-three chronic heart failure （CHF） in-patients were enrolled for investigation on quality of life. It was performed by filling in the Chronic Heart Failure Quality of Life Scale of Integrated Chinese and Western Medicine and SF-36. Cronbach＇s α coefficient and split-half coefficient were used to evaluate the reliability of both scales. The feasibility was decided by the fulfilling time and accomplishing rate of the scales. And the responsibility was dependant on the changes of quality of life scores of the patients before and after treatment. Results The total Cronbach＇s α coefficient of SF-36 was 0.896 and the split-half coefficient of total score was 0.903. The total Cronbach,s α coefficient of the Chronic Heart Failure Quality of Life Scale of Integrated Chinese and Western Medicine was 0.951 and the Split-half coefficient of total score was 0.960. The average responding time of SF-36 was （10.9±4.6）min,and that of Chronic Heart Failure Quality of Life Scale of Integrated Chinese and Western Medicine was （12.0±4.9）min. The accomplishing rate of the scales was both 96.83%. The difference of total score of quality of life of the scales before and after treatment was statistically significant （P0.01）. Conclusion Compared with SF-36,the Chronic Heart Failure Quality of Life Scale of Integrated Chinese and Western Medicine is good on reliability and responsibility. It＇s practical,so can be used as an instrument to measure the quality of life for chronic heart failure.
