双瓣置换加冠状动脉搭桥术(附1例报告)

A Successful Operation of Double Vavle Replacement Associated with Coronary Artery Bypass Grafting

报告１例风湿性瓣膜病合并冠心病患者在全麻低温体外循环下， 采用冷血间断灌注保护心肌行双瓣置换加冠状动脉搭桥术。术后２０ 天康复出院， 心功能由Ⅳ级上升到Ⅱ级。认为该病尽管病情复杂， 手术并发症多， 手术死亡率高， 但只要重视术前准备， 完善术中手术技巧和心肌保护措施， 提高术后监护水平， 可取得良好的手术效果。

Aim:to evaluate the outcome of combined double valve replacement and coronary artery grafting in patient with rheumatic valvular heart disease associated with coronary heart disease Method:The patient received cardiac surgery during general anesthesia,mild hypothemia and cardiopulmonary bypass which included mitral valve replacement,aortia valve replacement and coronary artery bypass grafting Result:the patient recovered with improved heart function from Ⅳ to Ⅱ grade Conclusion:Rheumatic valvular heart disease associated with coronary heart disease is correlated to high mortality rate If the coronary heart disease is diagnosed and treated actively preoperation,if surgical skill、myocardiac protection and postoperative care is improved,the excellent result may be received Coronary angiography should be carried out routinely in patients with vavular heart disease,especially in those patients over 50 years
