腰椎后路术后引流管放置与腰神经后支的关系

The relationship between the placement of drain and the position of posterior rami

目的 :为腰椎后路术后引流管放置提供解剖基础。方法 :解剖观察了 2 8例成人腰神经后支外侧支走行 ,深筋膜浅出点与后正中线的距离及相邻上下外侧支之间的距离。结果 :外侧支在深筋膜的浅出点集中于肩胛下角至骶髂关节连线的附近 ,其与后正中线距离的均数在 5cm以上。结论 :根据本文结果引流管出口宜在距后正中线 5cm以内 ,避开后支外侧支浅出处 ,以免形成疤痕或刺激神经 ,导致术后腰背疼痛。

To provide anatomy data for the proper placement of drain after lumbar surgery through posterior approach in order to prevent the irritation of posterior rami.Methods:The postero lateral branches of lumbar spinal nerve on both sides from T12 to L5 were dissected on 28 human cadavers.Results:The apertures of postero lateral branch of the lumbar nerve in fascia were situated around a line between the inferior angle of the scapula and the ipsilateral sacroiliac joint.The distance between the apertures and spinous processes was a little longer than 5 cm.Conclusion:The exit of the drainage tube should not be over 5 cm away from the spinous process and the 'T' type drainage tube was suggested.
