摘要 目的探讨PET—CT应用于前列腺癌诊断及分期的临床价值。方法2008年1月至2011年1月新疆自治区人民医院泌尿外科收治前列腺肿瘤病例中,病理类型最终确诊为前列腺癌者有47例接受了全身PET—CT检查。检查范围包括前列腺原发肿瘤、区域淋巴结及全身脏器,将PET—CT结果参照手术/病理结果进行评价分析。结果47例患者中1例未发现原发病灶,此外PET—CT与常规检查各有1例假阴性,准确性均高达95.7%,差异无统计学意义。35例I、Ⅱ期患者中18例髂血管淋巴结转移,PET-CT检查的敏感性59.6%、特异性81%、准确性70.3%,优于B超、磁共振检查;PET—CT发现4例患者合并远处转移并经穿刺证实,准确性100%。结论PET—CT对于发现前列腺癌原发肿瘤、区域淋巴结转移均优于B超、磁共振检查,且对于远处转移具有较精确的检测能力,对于前列腺癌术前诊断及分期有较好的临床价值。 Objectives To discuss the value of PET - CT application to prostate cancer diagnosis and the clinical stages of prostate cancer. Methods The patients with prostate tumors patients were Treated in people's hospital of xinjiang autonomous region Urology Surgery from January 2008 to January 2011. 47 patients who the final Pathological diagnosis of prostate cancer have accepted PET - CT systemic examination. Examination scope includes prostate primary tumors, regional lymph nodes and systemic organs, To analysis the results of PET - CT and surgery/pathologic. Results One case posteromedial were not found in 47 patients , moreover each of them have false negative in PET - CT and the conventional check , accuracy 95.7%, as no statistically significant differences. 18 patients have found metastasis to iliac blood vessels lymph node in 35 cases with Ⅱ period Ⅰ Its sensitivity to 59. 6%, specificity 81%, accuracy 70.3 % of PET - CT examination, more better than B ultrasonic and magnetic resonance imaging; four patients have been found distant metastasis with PET - CT examination and confirmed by the pathology, accuracy 100%. Conclusions Its better of PET - CT than B ultrasonic and magnetic resonance imaging, for discovering primary tumors, regional lymph node metastasis and to distant metastasis about prostate cancer. The PET - CT has a precise detection ability and good clinical value for prostate cancer preoperative staging and diagnosis.
