早期生大黄治疗对重度颅脑损伤患者TNF-α、IL-6含量的变化与临床意义

The Concentration of TNF-α and IL-6 in Peripheral Blood and the Clinical Significance of Early Nasogastric Feeding Rhubarb Powder on Patients with Severe Brain Injuries

目的:探讨早期生大黄治疗后重度颅脑损伤患者外周血TNF-α,IL-6含量的变化及临床意义。方法:将52例重型颅脑损伤患者随机分为生大黄组27例,对照组25例;两组基础治疗相同,生大黄组伤后24h给予生大黄粉经鼻胃管鼻饲,每次0.1g/kg,监测两组患者伤后24h、第3天、第14天的外周血TNF-α、IL-6含量、治疗间期的GCS变化。结果:生大黄组的TNF-α、IL-6含量低,且下降明显,与对照组相比有显著差异性(P<0.05),GCS相比有显著差异性(P<0.05)。结论:在重度颅脑损伤早期生大黄治疗可以抑制炎症介质的释放,防止肠道细菌移位及调节免疫抑制,防止继发性脑损害,在重型颅脑损伤的治疗中有着重要意义。

Objective ： To study the concentration of TNF - ct and IL - 6 in peripheral blood and the clinical significance of early nasogastric feeding rhubarb powder on patients with severe brain injuries . Methord ：52 patients with severe brain injuries were randomly divided into experimental group （27 cases） ,and control group （25 cases） . The fundermen- tal therapy of the two groups was same . The experimental group was treated by nasogastric feeding rhubarb powder 24 hours after brain injuries . The dose was 0.1 g / kg each , while the control group was not treated with rhubarb powder. Monitering the concentration of TNF - α and IL - 6 in the patients＇ peripheral blood and the change of Glasgow Coma Scale （GCS） ,during the process of therapy in 24 hours,the third day and the fourteenth day. Result ： The result indica- ted that the concentration of TNF - α and IL - 6 in experimental group was lower and decended apparently , compared with control group . There were significant differences in two groups （ P 〈 0.05 ） , and so were GCS （ P 〈 0.05 ） . Conclusion ： Early nasogastric feeding with rhubarb powder , which was an important theraphy in patients with severe brain injuries ,cart inhibit the releasing of inflammation mediators, prevent the displacement of enteric flora , regulate im- munological function and prevent secondary brain damage .
