血管内皮生长因子在慢性乙型肝炎患者血清中的表达及临床意义

Expression and Clinical Significance of Serum VEGF in Patients with Chronic Hepatitis B

目的:探讨血管内皮生长因子(VEGF)在慢性乙型肝炎(CHB)发病中的作用,旨在为CHB的诊治提供理论参考。方法:120例患者根据临床分型分为:慢性肝炎轻度30例,慢性肝炎中度43例,慢性肝炎重度47例,采用ELISA法测定120例慢性乙型患者和40例正常对照组血清VEGF水平,采用全自动生化仪检测肝功能常规指标。结果:慢性乙肝组VEGF、TB IL、ALT水平显著高于正常对照组,差异具有统计学意义(P<0.05);慢性乙肝组PTA水平显著低于正常对照组,差异具有统计学意义(P<0.05);慢性乙肝患者血清VEGF、TB IL水平随病情加重而呈上升趋势,PTA水平随病情加重而呈下降趋势,在轻、中、重度组间比较有统计学差异(P<0.05),而ALT水平在在轻、中、重度组间比较差异无统计学意义(P>0.05);慢性乙肝患者的VEGF水平与TB IL呈显著正相关(r=0.831,P<0.01),与PTA呈显著负相关(r=-0.620,P<0.01),而与ALT无明显相关性(r=0.169,P>0.05)。结论:VEGF参与了慢性乙肝的发病,并且是慢性乙肝病情严重程度的重要指标之一。

Objective：To explore the clinical significance of vascular endothelial growth factor（VEGF） in patients with chronic hepatitis B（CHB）,and provide theoretical reference for the diagnosis and treatment of CHB.Method：120 patients were divided into mild CHB（30 cases）,moderate CHB（43 cases） and severe CHB（47 cases） according to the clinical classification.Serum VEGF levels of 120 cases of CHB and 40 cases of healthy individuals were detected by electro-chemiluminescence immunoassay.The conventional parameters of liver function were detected by automatic biochemical analyzer.Result：VEGF,TBIL and ALT levels of CHB group were significantly higher than those in control group,whereas PTA level of CHB group was significantly lower than those of control group（P〈0.05,respectively）.VEGF and TBIL levels of CHB patients increased with the disease progression,and significant difference was found among the three groups of mild,moderate and severe group（P〈0.05）;PTA level of CHB patients decreased with the disease progression,and there was significant difference among the three groups（P 〈0.05）,but ALT level of CHB patients showed no significant difference among the three groups（P〈0.05）.VEGF level was positively correlated with TBIL level（r = 0.831,P〈0.01） and negatively correlated with PTA level（r =-0.620,P〈0.01）,but it was not correlated with ALT level（r = 0.169,P〉0.05）.Conclusion：VEGF involved in the pathogenesis of CHB is an important predictor for evaluating the severity of CHB.
