主动脉夹层DeBakeyI型患者临床特征分析

The Analysis of Clinical Character of Patients with DeBakey I Aortic Dissection

目的:分析主动脉夹层(Aortic dissection,AD)DeBakeyI型患者的临床特征,提高急诊确诊率。方法:回顾分析2007年1月至2010年1月间急诊收入的52例DeBakeyI型AD患者的临床特征。结果:52例DeBakeyI型AD患者突发胸骨后撕裂样剧痛为92.31%,有高血压史为59.62%,吸烟占28.85%,马凡综合症为13.46%;年龄分析年轻人多,男性多于女性。职业分析工人和农民等体力劳动者占73.08%;多于冬春季发病(76.92%)。发病24h内来诊患者D二聚体(D-dimmer,DD)水平高于24h后就诊的患者。外科手术治疗为81%。结论:DeBakeyI型AD患者,发病症状、年龄、职业及危险因素在临床上有其特殊性,手术治疗可改善预后。

Objective：To improve the diagnosis of emergency aortic dissection（AD）,through studying the clinical characteristics of DeBakeyI AD patients.Method：The data of 52 cases of DeBakeyI AD were obtained from the emergency department patients in general PLA hospital from Jan.2007 to Jan.2010.Result：Sudden chest pain was the first complaint of 92.31% patients.52 DeBakeyI AD patients with hypertension was 59.62%.Smoking history and Marfan syndrome was 28.85% and 13.46%.And the most part of the occupation of the 52 DeBakeyI AD patients was worker and farmer（about 73.08%）.Male more than female;The onset was almost in Spring and Winter（76.92%）.The D-dimmer was much higher in 24 hours than over 24 hours（P〈0.01）.Conclusion：To improve the recognition the symptoms,occupation,and risk factors of DeBakeyI AD patients,the identify rate of the AD will increase,and the prognosis will improve.
