摘要
目的探讨高原地区高血压患者血压升高与血尿酸的关系。方法选择2007年1月至2010年1月在我院内科住院治疗的世居或移居西藏高原或已从西藏退休定居在平原地区的高血压患者134例(高血压病组)。同期来我院体检的世居或移居西藏高原地区的健康者113例作为对照组。247例受试者均空腹12小时以上,于清晨抽取静脉血2~3 ml送检比较。结果高血压病组血尿酸水平显著高于对照组,差异有极显著的统计学意义(P〈0.001);高血压病1、2、3级各组血尿酸水平均显著高于对照组,差异有统计学意义(P〈0.05),1级患者血尿酸水平显著低于高血压病2级、3级(P〈0.001、P〈0.05),2级、3级高血压患者血尿酸水平相近,差异无统计学意义(P〉0.05);两组男性血尿酸水平均显著高于女性(P〈0.05、P〈0.001),汉族血尿酸水平均显著高于藏族(均P〈0.05)。结论血尿酸增高既是高血压病产生的原因之一,也是高血压病患者代谢紊乱的结果之一,产生的原因主要与血尿酸生成增加和排出减少有关。男性血尿酸水平均高于女性,汉族高于藏族。改善生活方式和饮食习惯,平衡持久的降压治疗,对改善体内代谢、降低血尿酸水平是有益的。
Objective To discuss the relationship between hypertension and blood uric acid of patients in plateau area.Methods We chose 134 cases of hypertension patients from Tibet treated in our hospital from January 2007 to January 2010 as observation group,and 113 controls who took physical examinations in our hospital at the same time.All the subjects were fasting for 12 hours or more.About 2~3 ml venous blood was taken for inspection.Results Total and 3 levels of blood uric acid in observation group were significantly higher than those in control group(P0.001,P0.05,P0.05,P0.05).Blood uric acid of hypertensive patients in level 1 was significantly lower than that of hypertensive patients in level 2 and 3(P0.001,P0.05).There was no significant difference in blood uric acid between level 2 and 3 hypertensive patients(P0.05).Blood uric acid of male was significantly higher than that of female in both observation group and control group(P0.05,P0.001).Blood uric acid of Han people was significantly higher than that of Tibetan(P0.05).Conclusions The increase of blood uric acid is one of the causes of hypertension,and also one of the results from metabolism in hypertensive patients.The main causes are the increased production of blood uric acid and the decreased discharge of it.Blood uric acid of male is higher than that of female,and blood uric acid of Han people is higher than that of Tibetan.It is suggested to improve the lifestyle and eating habits.Balanced and lasting antihypertensive therapy is beneficial for improving metabolism and reducing blood uric acid level.
