负压封闭引流与闭式灌洗引流治疗慢性化脓性骨髓炎的近期疗效比较

Effect of vacuum sealing drainage and closed continuous douche for chronic pyogenic osteomyelitis

目的比较负压封闭引流术（VSD）与传统闭式灌洗引流术（CCD）治疗慢性化脓性骨髓炎的疗效。方法 18例慢性化脓性骨髓炎患者经彻底清创后分别给予VSD和CCD治疗,观察两组疗效。结果 VSD组10例,随访11-32个月,平均20.7个月;CCD组8例,随访13-27个月,平均20个月。两组患者症状复发率分别为10%和25%,经再次使用VSD治疗后均好转,两组复发率比较差异有统计学意义（P〈0.05）。结论与传统CCD相比,VSD治疗慢性化脓性骨髓炎可以降低复发率,提高治愈率,不足之处是费用相对较高。

Objective To compare the effect of vacuum sealing drainage（VSD）and closed continuous douche（CCD）for chronic pyogenic osteomyelitis.Methods Eighteen patients suffered chronic pyogenic osteomyelitis were divided into two groups after thorough debridement,and were given VSD and CCD therapy respectively.The effect of two groups were observed and analyzed.Results The 10 cases in VSD group were followed up for 11 to 32 months（20.7 months on average）,while the 8 cases in CCD group were followed up for 13 to 27 months（20 months on average）.The recurrence rates of two groups were 10% and 25% and the difference was significantly different（P0.05）.All the recurrence patients accepted VSD therapy and recovered.Conclusions Compared with traditional CCD method,VSD technology could reduce the recurrence rate of chronic pyogenic osteomyelitis and increase the recovery rate.But the cost of VSD is much higher than that of CCD.
