摘要 目的评价CT引导下经皮肺穿刺活检的临床应用价值。方法采用自动活检切割针(18G)在CT引导下对153例患者肺部病灶行经皮肺穿刺活检术。结果 153例均穿刺成功,146例取得满意标本,7例为坏死组织未能做出诊断;穿刺活检的129例恶性肿瘤患者中有48行手术治疗,其术后病检与穿刺活检结果一致;7例炎症;5例结核;4例真菌;有36例出现了轻重不同的并发症,气胸32例(20.9%),3例(1.9%)出现少量咯血或痰中带血,1例发生胸腔少量出血,经相应治疗后病情好转出院。结论 CT引导下经皮肺穿刺活检诊断肺外周病灶并发症少、安全有效,具有较高的临床应用价值。 Objective To evaluate the clinical value of CT guided percutaneous lung biopsy.Methods Lung lesions of 153 patients underwent percutaneous lung biopsy by automatic cutting needle biopsy(18G).Results A total of 153 patients were punctured successfully,including 129 cases of cancer(48 cases had consistent pathologic and biopsy results),7 cases of inflammation,7 cases of necrotic tissue,5 cases of tuberculosis and 4 cases of fungus.Thirty-six patients had compilactions,including 32 cases of pneumothorax(20.9%),3 cases of hemoptysis or bloody sputum(1.9%)and 1 case of pleural bleeding.All the patients were discharged after treatment.Conclusion CT-guided percutaneous needle biopsy of lung lesions is safe and effective,with high diagnosis rate,fewer complications and high clinical value.
