经尿道膀胱颈部分切开术治疗慢性前列腺炎

Transurethral bladder neck incision of parts for the treatment of chronic prostatitis

目的:探讨经尿道膀胱颈部分切开术治疗慢性前列腺炎的方法及价值。方法:从2008～2010年采用经尿道膀胱颈部分切开术16例,患者病史均为1年以上,术前常规行细菌学检查,给予足量、足疗程抗生素治疗,患者尿频、尿急、尿痛症状无好转,部分患者出现尿潴留症状,严重影响患者的生活质量及心理健康。术中采取切开膀胱颈3～9点处膀胱颈前列腺组织至精阜平面。结果:16例患者手术成功;手术时间30～70min,平均45min;除1例发生5d大出血,再次留置尿管持续膀胱冲洗等治疗后好转,余患者1周后拔尿管出院,术后继续服用抗生素4～6周,1例患者6周后症状无好转,继续服药至3个月,患者仍有尿频,尿痛及会阴部疼痛好转,1例患者术后1年后出现尿道狭窄,再次行膀胱颈切开治疗,术后随访3～18个月,患者尿频、尿急、会阴区胀痛等症状明显好转,无严重并发症出现。结论:经尿道膀胱颈部分切开术治疗慢性前列腺炎效果肯定,可作为慢性前列腺炎患者的治疗方法之一。

Objective：To discuss the transurethral bladder neck in the treatment of chronic prostatitis techniques and clinical value.Methods：From 2008 to 2010,use of transurethral bladder neck minute incision 16 cases.Patients medical history were more than 1 year,bacteriological examination before the regular line given enough,foot treatment antibiotic therapy,patients with urinary frequency,urinary urgency,the symptoms do not improve niaotong,some symptoms in patients with urinary retention,seriously threatened the quality of life of patients and health.Take in incising bladder neck 3 to 9 point bladder neck prostate tissue to verumontanum plane.Results：The operation was successful in 16 patients,the operation time ranged from 30 to 70 minutes,mean 45 minutes,blood ocurred in one patient after 5 days of operation,once again,indwelling catheter,continuous bladder irrigation improves after treatment,more than 1 week after patients with catheter discharged from hospital,4-6 week continues to take antibiotics after the surgery,one patient do not improve after six weeks,continue taking up to 3 months continue taking up to 3 months,in patients with urinary frequency in patients with urinary frequency,but urinary pain and perineal pain improved.One patient 1 year after the operation occurs urethral stricture,again to the bladder neck incision therapy.Postoperative follow-up 3 to 18 months,16 patients with urinary frequency,urinary urgency,perineal pain and other symptoms significantly improved,no serious complications occur.Conclusion：Transurethral bladder neck in the treatment of chronic prostatitis effect positive,can be used as a treatment for chronic prostatitis methods.
