应用20项新生儿行为神经测定影响因素分析 被引量:14

摘要 为进一步了解新生儿窒息,低体重儿、剖宫产儿对神经系统的损伤情况,按“中国新生儿20项行为神经评分法”在生后第7天、第14天、第28天进行3次测试,对50例自然分娩的正常新生儿、50例剖宫产儿、30例轻度窒息儿及15例低体重儿分别进行测定。结果,轻度窒息与正常新生儿比较差异有显著意义(P<0.01)。此外,轻度窒息组第28天第3次复测与生后第7天测定,差异也有显著意义。低体重儿与正常新生儿比较,3次测定均有显著意义(P<0.01)。但其脑的功能,运动能力随着日龄的增加逐渐恢复。剖宫产组生后第7天、第14天测试与正常新生儿比较差异有显著意义(P<0.01)。但第28天复测与正常新生儿第28天比较无明显差异(P>0.05)。新生儿窒息、低体重儿是影响新生儿脑功能、运动能力的一个重要因素,早期干预有利于促进受伤大脑的功能代偿。 In order to know the trauma of newborn's asphyxiation, lower body weight newborn,and abdiominal newborn on the nerve system, according to the 'Chinese Newborn's Action Nerve Grade Method of 20 Items', three detection were carried out at the 7th day, 14th day,28th day after birth. 50 normal newborns of the natural delivery, 50 abdiominal newborns,30 low-grade asphyxiation newborns and 15 lower body weight newborns were detected respectively. The results showed that the low-grade asphyxiation newborn had the distinct difference comparing to the normal newborn (P<0.01). Besides, the 3rd redetection of the low-grade asphyxiation group at 28th day also had the distinct difference comparing to the detection at the 7th day. Three detection of the lower body weight newborn had the distinct difference comparing to the normal group (P <0. 01 ), but their brain's function and movement ability would regain gradually with the increase of the days. The detection of the abdiominal newborn at 7th day, at 14th day had the distinct difference comparing to the normal group (P<0.01), but the redetection at 28th day had not the distinct difference comparing to the normal group at the 28th day (P >0. 05). Newborn's asphyxiation and lower body weight was an important factor, which influenced the brain function and movement ability of the newborn. The early intervene would be in favor of promoting the trauma brain function.
