内镜下粘膜切除术的围手术期护理

Perioperative nursing of endoscopic mucosal resection

目的探讨胃肠壁病变内镜下粘膜切除术(EMR)的围手术期护理价值。方法对146例需行EMR的患者术前做好心理护理与手术准备;术中医护人员熟练配合;术后加强并发症的观察,正确指导饮食。并对手术结果进行分析。结果在146例患者中,141例安全、成功地完成了EMR治疗,手术成功率96.6%;5例因病灶过大、位置过深、部位不恰当或隆起不充分而失败改为外科手术。除3例发生创面脱痂出血外,其余均无出血、穿孔、强烈疼痛等并发症发生。结论内镜下粘膜切除术治疗胃肠壁病变具有创伤小、疗效好的优点,加强围手术期护理对提高手术的安全性及成功率具有重要的临床价值。

Objective To explore the methods of perioperative nursing of endoscopic mucosal resection（EMR）.Methods Patients required EMR treatment were disposed of the following measures including before surgery,good psychological care,good preoperative preparation;during the operation,with skilled health care workers;in surgery to strengthen the complications,the correct post-operative dietary guidance.The surgical results were analyzed.Results Among146 patients,141 cases were safe and successful completion of injection of EMR,the success rate was 96.6%.The lesions of 5 cases were large,deep and inappropriate or inadequate and fail to uplift surgery.Conclusion EMR for gastrointestinal wall lesions is with less trauma and better efficacy advantages of enhanced care to improve surgical safety and critical success.
