20例外阴癌电子束照射

VULVA CANCER TREATED WITH HIGH ENERGY ELECTRONS:AN ANALYSIS OF 20 CASES

目的 研究采用高能电子束照射治疗外阴癌的疗效。 方法 ２０ 例外阴癌中初治组８ 例，局部病灶照射总量为６０ ～６５ Ｇｙ／５～６ 周，腹股沟区照射４０ Ｇｙ／４ ～５ 周。术后复发组７ 例，局部病灶照射总量６０ ～６５ Ｇｙ／５ ～６周。术后肿瘤残留补充放疗组５ 例，局部照射３０ ～４０ Ｇｙ／３ ～４ 周。 结果 照射后肿瘤全部消退占７３３３ ％ （ 不包括术后补充放疗组在内），皮肤反应较６０Ｃｏ 为轻，三年生存率为６１５％ 。 结论 有手术禁忌证者，较表浅的肿瘤，术后切缘有癌，术后复发者均可采用放疗，如并用以顺铂为主的化疗可提高疗效。

To study effectiveness of high energy electrons in vulva cancer.METHODS From December 1986 to February 1995,20 cases of vulva cancer were treated with high energy electrons in our hospital. Of the 20 cases , 8 received conventional radiotherapy with a dose of 60-65 Gy/5-6wks to primary tumor and 40 Gy/4-5wks to inguinal region.7 cases with post operative recurrence received radiation of 60-65 Gy/5-6wks to local lesion and 5 cases with residual tumor after surgery received further radiotherapy of 30-40 Gy/3-4 wks to residual site. RESULTS Complete regression after radiotherapy (not inculding patients with residual tumor after surgery) was 73.33%. Skin reaction was milder than after 60 Co radiation. The three year survival rate was 61.5%.CONCLUSIONS It is suggested that patients who are inoperable or with advanced disease, positive surgical margin and recurrence after surgery should receive radiotherapy,and if combined with chemotherapy eg. DDP. the results may be further improved.
