严重虹膜后粘连的并发性白内障摘出人工晶状体植入术

Extracapsular cataract extraction and intraocular lens implantation for complicated cataract with serious posterior synechiae of iris

目的 探讨减少严重虹膜后粘连的并发性白内障囊外摘出 (ECCE)人工晶状体 (IOL)植入术后的并发症。方法 对 14例 ( 17只眼 )伴有严重虹膜后粘连的并发性白内障 ,不强行分离虹膜后粘连 ,采用垂直径线剪开瞳孔上下方虹膜达中段 ,线状截囊 ,囊袋内晶状体摘出及IOL植入。结果 术后随访 6～ 2 5个月 ,无严重并发症发生 ,术后视力 0 .5以上者达 2 9.4%。结论 对严重虹膜后粘连并发性白内障 ,术中采用的不强行分离粘连及囊袋内操作 ,可避免对虹膜的刺激 ,减轻术后虹膜炎症反应 。

Objective To decrease the complications after extracapsular cataract extraction(ECCE) and intraocular lens(IOL) implantation for complicated cataract with serious posterior synechiae of iris. Methods Without separating synechiae of iris firmly,operation included iridectomy through pupil at vertical meridian,ECCE and implantation of IOL in the bag. Results After a follow up of 6～25 months,no serious complications occurred in these cases.The corrected visual acuity in 29.4% of the patients reached 0.5 or better. Conclusion The operation method of not separating synechiae and manipulation in the bag can avoid excessive stimulation of iris,reduce postoperative inflammation reaction of iris,and prevent iris adhesion with IOL.
