摘要
【目的】了解泛耐药鲍曼不动杆菌(PDR-AB)10簇β-内酰胺酶(bla)基因、2簇碳青霉烯酶基因(OXA)、2簇氨基糖苷类修饰酶(AMEs)基因、3种整合子酶基因和消毒剂-磺胺类耐药基因携带情况。【方法】用聚合酶链反应(PCR)及序列分析方法对30株从我院分离的PDR-AB进行各种耐药基因的检测和分析。【结果】在30株PDR-AB中blaADC阳性30株(100%),blaSHV阳性29株(97%),blaVIM阳性26株(87%),blaIMP阳性24株(80%),blaPER阳性14株(47%),blaTEM阳性14株(47%),blaDHA阳性12株(40%),blaGES阳性8株(27%)b,laGIM阳性6株(20%);OXA-23阳性27株(90%);Ant(3’’)-I阳性30株(100%)I;ntI1阳性30株(100%)I,ntI2阳性8株(27%)I,ntI3阳性2株(7%);QacE△1-sull阳性30株(100%);没有检测到blaVEB、OXA-24和Aac(6’)-II基因。【结论】泛耐药鲍曼不动杆菌同时携带多种耐药基因是导致其对所有常用药物均耐药的重要原因,临床应加强有效的监测和隔离工作来控制其传播。
[Objective] To investigate the existence of the genes encoding ten beta-lactamases(bla), two carbapenemases(OXA), two aminoglycosides-modifying enzymes(AMEs), genes of three integrases and disinfectants-sulfanilamide-resistance gene in pan-drug resistant Acinetobacter baumannii(PDR-AB). [ Methods] Genetypes of thirty strains of PDR-AB were analyzed by polymerase chain reaction(PCR)and verified by DNA sequencing. [ Results ] The positive rates of blaADC, blaSHV,blaVIM ,blaIMP,blaPER,blaTEM, blaDHA , blaGES, blaGIM, OXA-23, Ant(3 ' ' )-I, IntI 1, IntI2, IntI3 and QacE △ 1 -sull were respectively 100%, 97% 、 87%、 80% 、47%、 47% 、40%、 27%、20%、 90%、100%、100%、 27%、 7% and 100%, blaVEB, OXA-24 and Aac(6 ')-II weren' t detected. [ Gonclusion ] The pan-drag resistant Acinetobacter baumannii was cm'rying so many antiblotic-reslstance-related genes, which was the most important causing that led to resistance to all drugs used commonly. So effective monitoring and quarantine work should be strengthened to control the epidemic of these strains.
