非手术治疗强直性脊柱炎髋关节病的临床研究

Clinical study of non-surgical treatment of hip joint disease with ankylosing spondylitis

目的研究物理、康复与中药联合治疗强直性脊柱炎髋关节病的临床疗效。方法分析48例强直性脊柱炎髋关节病的患者,应用体外冲击波疗法、熏蒸疗法、中药疗法、传统中医康复手法进行治疗。所有病例分别在治疗前、治疗后3、6、9、12个月,进行关节Harris评分、Schober试验、ESR、BASFI、BASDAI和髋关节X线片检查。结果患者治疗后3、6、9、12个月髋关节Harris评分(疼痛、功能、畸形、活动范围)、Schober实验、ESR、BASFI、BASDAI与治疗前比较,各项指标均有良性改善,差异有统计学意义(P<0.05或P<0.01)。结论物理、康复与中药联合治疗能充分发挥各种治疗方法的特点,起到协同作用,对改善强直性脊柱炎整体病情及髋关节功能有较好的疗效,具有疗效显著、无创伤性等优点。

Objective To study clinical efficacy of the physical therapy,rehabilitation and traditional Chinese medicine to treat of hip joint disease with ankylosing spondylitis.Methods 48 cases patients of hip joint disease with ankylosing spondylitis were treated by combined methods,including extracorporeal shock wave therapy,fumigation therapy,traditional Chinese medicine,traditional rehabilitation.Bfore and after 3,6,9,12 months of treatment,Harris hip score,Schober test,ESR,BASFI,BASDAI and hip X-ray film were tested and recorded.Results Harris hip score（pain,function,deformity,range）,Schober test,ESR,BASFI,BASDAI were positive inprovement compared with before treatment,the differences were statistically significant（P0.05 or P0.01）.Conclusion The physical therapy,rehabilitation and traditional Chinese medicine will full play the characteristics of various treatments,and obtation significant effect on improving the overall condition of AS and function of hip joint disease,also had the advantages of non-invasive and treatment so on.
