脑干听觉诱发电位对急性一氧化碳中毒后迟发性脑病的预测价值 被引量:6

摘要 目的探讨脑干听觉诱发电位(BAEP)对急性一氧化碳中毒后迟发性脑病(DEACMP)的预测价值。方法对32例DEACMP患者(DEACMP组)及34例无DEACMP的急性一氧化碳中毒患者(非DEAC-MP组)进行BAEP动态检查,比较两组BAEP检查的结果,并与60名健康对照者(正常对照组)进行比较。结果首次BAEP检查发现,DEACMP组中18例(56.3%)异常,非DEACMP组仅3例(8.8%)异常;与非DEACMP组、正常对照组相比,DEACMP组除Ⅰ~Ⅲ波峰间期外,其余各波潜伏期及波峰间期均显著延长(均P〈0.01);非DEACMP组与正常对照组首次BAEP检查各波潜伏期及波峰间期比较差异无统计学意义。DEACMP组其后5次BAEP检查的异常率分别为12.5%、75.0%、9.4%、3.1%和3.1%,而非DEACMP组均为0。首次BAEP检查异常对DEACMP预测的敏感度为56.3%,特异度为91.2%。结论急性一氧化碳中毒患者早期BAEP检查异常可预测DEACMP的发生。 Objective To explore the predictive value of brainstem auditory evoked potential(BAEP) on delayed encephalopathy after acute carbon monoxide poisoning(DEACMP).Methods The dynamic examination of BAEP was underwent on 32 DEACMP patients(DEACMP group) and 34 acute carbon monoxide poisoning patients without DEACMP(non-DEACMP group).The results of BAEP in the two groups were compared with each other,and also were compared with 60 healthy controls(normal control group).Results In the first BAEP examination,18 cases(56.3%) in DEACMP group and 3 cases(8.8%) in non-DEACMP group were abnormal.Compared with non-DEACMP group and normal control group,the latencies and the durations of all the waves were significant longer except Ⅰ-Ⅲ wave duration in DEACMP group(all P0.01).The latencies and the durations of all the waves in the BAEP examination between non-DEACMP group and normal control group were no significant difference.In the later 5 times BAEP examination,the abnormal rates in DEACMP group were 12.5%,75.0%,9.4%,3.1% and 3.1%,respectively;but which in non-DEACMP group was all 0.The sensinbility of abnormal BAEP on DEACMP was 56.3%,and the specificity of which was 91.2%.Conclusion At the early stage,abnormal result of BAEP may have some predictive value on DEACMP in patients with acute carbon monoxide poisoning.
