摘要
目的报道1例肾原发性神经内分泌癌,并探讨其病理学特征及其鉴别诊断。方法对1例肾神经内分泌癌进行光镜及免疫组化标记,并结合文献复习。结果肿瘤位于肾中上极,大小为4.5 cm×3.5 cm×2.5 cm。光镜示以巢状结构为主,肿瘤细胞密集异型,小圆形及小梭形,胞质少,核富于染色质,核仁不明显,局部区域富于核分裂象,伴灶性出血、坏死。免疫组化示瘤细胞CgA、Syn、NSE、AE1/AE3和bcl-2(+)。结论肾神经内分泌癌是一种少见的恶性肿瘤,其诊断需结合组织形态及免疫表型。
Objective To study the clinical pathological features and differential diagnosis of renal neuroendocrine carcinoma.Methods A case of renal neuroendocrine carcinoma was investigated by light microscopy and immunohistochemistry,and the literature was reviewed.Results The tumor was located in the middle-upper pole of kidney.The tumor was 4.5 cm × 3.5 cm × 2.5 cm in size.Microscopically,the tumor was composed of intensive allotypic small round and spindle-shaped cells with little cytoplasm arranged in nests.Chromatin-rich nucleus was not obvious and karyokinesis,hemorrhage and necrosis were easily identified in the local area.Immunohistochemically,tumor cells were stained positively for CgA,Syn,NSE,AE1 /AE3 and bcl-2.Conclusion Neuroendocrine carcinoma of the kindey is a rare malignant tumor and the diagnosis needs to combine with pathomorphology and immunophenotype.
