腰骶部移行椎与腰椎管狭窄的相关性分析 被引量:12

Analysis of correlation between lumbosacral transitional vertebra and the lumbar spinal stenosis

摘要 [目的]探讨腰骶部移行椎和腰椎管狭窄的相关性。[方法]随机选取因各种原因行腰椎X线平片和CT扫描检查者121例,发现有移行椎(A组)34例,无移行椎(B组)87例,对两组CT扫描腰椎管狭窄各参数进行统计学对比分析,并对两组病变节段进行比较。[结果]有移行椎组合并腰椎管狭窄23例(占67.65%),无移行椎组发现腰椎管狭窄37例(占42.53%),两组之间存在显著性差异。A组椎间盘退变突出、黄韧带肥厚、小关节退变及侧椎管狭窄的发生率皆与B组存在显著差异,中央椎管狭窄的发生率无统计学差异。[结论]腰骶部移行椎可以影响腰骶椎的退行性改变,加速椎间盘退变突出、黄韧带肥厚、小关节退变及侧椎管狭窄,且退变多位于移行椎的上一间隙,对中央椎管的骨性狭窄无明显影响。 [Objective]To investigate the correlation between lumbosacral transitional vertebra(LSTV) and the lumbar spinal stenosis.[Methods]Totally 121 patients undergoing plain film and CT scan were randomly divided into 2 groups.Thirty-four patients with LSTV were in group A,and the others in group B.Some lumbar spinal stenosis parameters on CT scans were compared between groups.The degenenative level was also compared.[Results]Lumbar spinal stenosis was found in 23 in group A and 37 in group B,which had significant difference.There was statistically significant difference in the incidence of disc degeneration and herniation,ligamenta flava pachynsis,facet degeneration and lateral spinal stenosis between groups.The central spinal stenosis showed no statistical significance.[Conclusion]LSTV can influence lumbar degeneration including the intervertebra disc degeneration and herniation,ligamenta flava pachynsis,facet degeneration and lateral spinal stenosis.And these degeneration occurs more often at the level suprajacent to LSTV.But there is no obvious effect on the central bony spinal stenosis.
