分段双重挂线术治疗高位肛瘘的临床研究

Segmental Double Thread-drawing Procedure in the Treatment of High Anal Fistula：Clinical Study

为观察分段双重挂线术治疗高位肛瘘的临床疗效，将60例高位肛瘘患者分为研究组（30例）和对照组（30例）。研究组采用分段双重挂线术，对照组采用切开挂线术，以术后2d、术后7d、术后14d、术后3个月、术后6个月（随访记录）的临床症状及创面愈合时间、有无术后并发症等作为主要观察指标。结果显示，研究组能有效减轻术后疼痛、渗出等症状的程度，缩短手术创面愈合时间；远期随访（术后3个月、术后6个月），在疗效及术后并发症方面，两组差异无统计学意义（P〉0．05）。结果表明，分段舣重挂线术治疗高位肛瘘疗效可靠，能有效减轻术后症状，手术创面愈合时间短，瘢痕小，是一种行之有效的手术方法。

In order to investigate the clinical effect of the procedure of segmental double thread-drawing in the treatment of high anal fistula,authors divided the 60 patients into study group（ n =30） and controlled group（ n = 30）. In study group above-procedure was adopted, while in controlled group, incision and thread-drawing procedure;the main observation index for them included clinical symptoms and healing time of wound surface, as well with or without complication, etc at 2,7,14 days, at 3,6 months （according to follow-up record） after operation. As results,in study group,postoperative pain and exudation extents was effectively reduced,and with shorter healing time of operative wound surface; and, the long-term follow up （3,6 months after operation） did not find the statistical difference between both groups in the therapeutic effect and incidence of complication（ P〉0.05）. Results show that the segmental double thread-drawing procedure has certain therapeutic effect, can effectively relieve postoperative symptoms;its healing time of operative wound furface is shorter,scar also less,thus it is of effective operation method.
