亚洲钉治疗老年股骨粗隆间骨折的疗效分析 被引量:4

Analysis of Effect of Asian IMHS in Treatment of Intertrochanteric Fractures of Femur in Old People

摘要 目的探讨亚太型股骨近端髓内钉(Asian intramedullary hip screw,Asian IMHS,简称亚洲钉)微创治疗老年股骨粗隆问骨折的疗效.方法 自2008年3月-2009年3月采用亚洲钉治疗老年股骨粗隆间骨折57例。42例获得随访。分析患者的手术时间、术中出血量,随访骨折愈合情况,采用Harris评分评估患者的髋关节功能。结果42例获得3~15个月随访,平均8个月。平均手术时间为(63±12)min,平均出血量为(107±13)ml。35例达到骨性愈合;6例发生延迟愈合,其中1例因骨折端分离较重发生不愈合;1例因术中内翻畸形未获适当矫正导致拉力螺钉早期切出。术后髋关节Harris评分:优30例,良5例,可5例,差2例。结论采用亚洲钉治疗老年股骨粗隆间骨折具有操作简单、创伤小、骨折块间具有加压作用、固定可靠,是治疗此类骨折的较好方法. Objective To investigate the effect of Asian IMHS in treatment of the intertrochanteric fractures of femur in old people. Methods From March 2008 to March 2009, 57 patients with intertrochanteric fraetures of femur were treated with Asian IMHS. Forty two patients were followed up. All the fractures were treated by close-reduction and fixed with Asian IMHS. The operation time, volume of blood loss, fracture union time and hip function were analysed using Harrison score.Results A total of 42 patients were followed-up for 3-15 months with an average of 8 months. The operation time was (63±12)min, and the volume of blood lose was (107±13)ml. Thirty five cases came to synostoses; 6 cases, delayed union; 1 case, non-union because of the distraction; 1 case, the lag screw cut out as a result fi'om the improper modification. According to the Harris score, 30 patients obtained excellent function; 5, good function; 5, fair function; 2, poor function.Conclusion Asian IMHS is a good choice for treatment of the intertroehanteric fractures of femur in old people with easy performance, less blood loss and stable fixation.
