101份药历的用药分析

Analysis Drug Use on the 101 Copies of Pharmacy History

目的分析蚌医二附院临床药学室所建药历的用药情况,针对不合理用药情况提出相应干预措施,促进临床医师合理用药。方法采用回顾性分析方法,随机抽取本院2010年所建药历101份,分析、总结其用药情况,结合药品说明书、药物临床信息参考等资料判断所建药历的用药合理性,并提出解决办法。结果存在不合理用药情况的药历共24例,约占总数的24%,主要表现在重复给药、用法用量不正确、配伍不适宜、溶媒选择不适当、选药不合理、中药注射剂大于2种等方面,以选药不合理居首,占33%,少见重复给药和溶媒选择不正确。结论需进一步加强临床药学工作。药学人员深入临床参与查房,协助医、护及医院管理等工作,建立良好的监管机制,促使本院合理用药水平进一步提高。

Objective To analyze the drug use state of the pharmacy history created by clinical pharmacy room in our hospital,and put forward corresponding interfere measure so as to promote the level of rational drug use.Methods A retrospective analysis method was taken to analyze and summarize the drug use state of the 101 copies pharmacy history selected randomly,and combined with pharmacy description and medicine clinical information reference etc.data to determine the rationality of the drugs use.Results 24 copies of sampled pharmacy histories were found irrational,accounting for 24% of the total amount.The irrationality appeared mainly as repeated drug use,improper usage and dosage,unreasonable compatibility of medicines,unreasonable selection of solvent,unreasonable choice of drugs,traditional Chinese medicine injection more than 2 kinds etc.The common reason was unreasonable choice of drugs,accounting for 33%,repeated drug use and unreasonable selection of solvent were rarely seen.Conclusion Clinical pharmacy work needed to be strengthened further.Pharmacists should pay much more attention to go deep among the clinical practice and participate in clinical rounds,then assisted doctors,nurses and hospital managements to set up good governance mechanism for promoting the level of rational drug use.
