摘要 烧伤休克延迟复苏,是指烧伤休克已经发生并持续了一段时间后才开始的液体复苏治疗,往往得不到预想的疗效,在后续病程中多脏器功能衰竭的发生率和病死率都很高。本文通过大量查阅近年来与烧伤休克延迟复苏相关的文献,总结了烧伤休克延迟复苏的定义和临床特点,阐述了其引起重要脏器细胞损伤的三大主要机制——细胞内钙超载、氧化应激和蛋白酶异常活化,并在此基础上综述了近年来针对上述三大损伤机制的新的措施理念和新研发药物的相关进展。 Delayed fluid resuscitation is the resuscitation which starts after severe thermal shock has happened and lasts for some time.Its effects are not quite satisfactory,and during the continuing process,the incidence of multiple organs failure and mortality is considerable high.In this article,we reviewed lots of commentaries concerning the delayed fluid resuscitation,through them,we summarized the definition and the clinical characteristics of it.Moreover,we described its main mechanisms(intracellular calcium overload,oxidative stress and abnormal activation of protease) which are causing the terrible injury of the cells and vital organs.On these bases,we discussed some developing measures and advanced drugs which are towarding the mechanisms of the delayed fluid resuscitation.
