摘要
目的:探讨使用高频彩色多普勒超声技术和X线钼靶检测提高早期乳腺癌的诊断率。方法:运用X线钼靶和高频彩色多普勒超声技术及两种方法联合对97例早期乳腺癌患者进行全面检查,与病理检查结果对照分析。结果:经高频超声诊断恶性病变66例,漏误诊14例,其敏感性、特异性、准确性分别为88.2%(15/17)8、2.5%(66/80)、83.5%(81/97),经钼靶x线诊断恶性病变63例,漏误诊17例,敏感性、特异性、准确性分别为70.6%(12/17)、78.8%(63/80)、77.3%(75/97)。两者联合诊断恶性病例75例,其敏感性、特异性、准确性分别为94.1%(16/17)9、3.8%(75/80)、93.8%(91/97)。彩超敏感性和特异性高于X线,但差异无统计学意义(P>0.05),彩超检测的准确性明显优于X线(P<0.05)。X线和超声联合使用诊断的敏感性、特异性、准确性与单独使用超声或X线相比均有显著差异(P<0.05)。结论:两种方法联合应用可明显提高乳腺癌的诊断准确率,敏感性及特异性性,有较高的临床价值。
Objective: To investigate the predictive value of high frequency ultrasound(HFUS),molybdenum target x-ray in the diagnosis of early stage breast cancer.Methods: Ninety-seven patients with early stage breast cancer underwent HFUS and molybdenum target x-ray detection.The diagnosis results were compared with that of pathological examination.Results: The sensitivity,specificity,accuracy of HFUS,molybdenum target x-ray and the combination of two was 88.2%(15/17)、82.5%(66/80),83.5(81/97) vs.70.6%(12/17)、78.8%(63/80)、77.3%(75/97) vs.94.1%(16/17)、93.8%(75/80)、93.8%(91/97).The sensitivity and specificity of x-ray was higher than that of the HFUS but no significant difference was found(P0.05),the accuracy of x-ray was significant higher than that of the HFUS(P0.05).The combined use of the two methods showed higher sensitivity,specificity,accuracy than one method alone.Conclusion: High-frequency ultrasound combined with molybdenum target x-ray shows higher accuracy,specificity and sensitivity in detection of early stage breast cancer,which is of great clinical significance.
