乙状窦憩室引起搏动性耳鸣的CT血管造影诊断

CT arteriography and venography diagnosis in pulsatile tinnitus caused by sigmoid sinus diverticulum

目的评价CT血管造影在诊断乙状窦憩室异常引起搏动性耳鸣的应用价值。方法搜集12例乙状窦壁异常所致搏动性耳鸣患者,回顾性分析CT血管造影表现。结果 12例乙状窦壁异常引起搏动性耳鸣患者中右侧9例,左侧3例。CT血管造影表现为乙状窦前外壁或外壁骨质缺损,边缘光整的软组织团块突入乳突蜂房内,其中2例突出至皮下,突出的软组织与乙状窦相延续且密度相等。结论 CT血管造影检查对于乙状窦壁异常所致搏动性耳鸣的诊断及鉴别诊断具有重要价值,但临床仍需结合耳鸣特点选择检查。

OBJECTIVE To evaluate the diagnostic value of CT arteriography and venography in pulsatile tinnitus caused by sigmoid sinus diverticulum.METHODS Retrospectively studies of CT arteriography and venography imaging of 12 cases of pulsatile tinnitus caused by sigmoid sinus diverticulum.RESULTS 12 cases of pulsatile tinnitus caused by sigmoid sinus diverticulum were identified,in which nine of twelve were right side,and three were left side.CT showed bone defects of the wall of sigmoid sinus combined with invasion of soft tissues mass which was continued and isodensitied with sigmoid sinus in mastoid cells.CONCLUSION CT arteriography and venography can be valuable imaging tools in the diagnose and differential diagnose of pulsatile tinnitus caused by sigmoid sinus diverticulum.With this technique,arterial,venous,middle,and inner ear causes of PT can be excluded,however the clinicians should identify the characteristic of tinnitus before choose the protocol.
