改良腮腺浅叶切除术治疗腮腺良性肿瘤

Modified superficial parotidectomy for benign parotid gland tumor

目的探讨腮腺良性肿瘤切除术中保留腮腺咬肌筋膜、耳大神经和腮腺导管对降低术后并发症的作用。方法回顾性分析56例腮腺良性肿瘤行肿瘤切除术的患者,保留腮腺咬肌筋膜、耳大神经和腮腺导管,术后予负压引流和局部加压包扎,观察并发症的发生率。结果所有患者术后均随访,无1例复发。3例患者发生面神经损伤,均为一侧口角略歪斜,鼓腮稍漏气,6例患者出现短暂性耳垂麻木以及术区局部出现轻度凹陷畸形。结论腮腺浅叶切除术中保留腮腺咬肌筋膜、耳大神经和腮腺导管,可有效减少术后并发症的发生。

OBJECTIVE To investigate the advantage of preserving the parotid masseter muscle fascia,great auricular nerve and Stensen＇s duct in modified superficial parotidectomy.METHODS The parotid masseter muscle fascia,great auricular nerve and Stensen＇s duct were preserved during partial parotidectomy in 56 cases with benign parotid gland tumor.All of the patients were received negative pressure drainage and local pressure dressing after the surgery.RESULTS All cases had a follow-up after operation,no recurrence occurred.Facial nerve impaired was developed in 3 cases,all of them were slightly unilateral drooping of the angle of the mouth,and developed gas leakage when air-blowing.Six cases developed ear lobe numb syndrome temporarily.All of the cases developed local mild hollow malformation in the operation area.CONCLUSION Preserving parotid masseter muscle fascia,great auricular nerve and Stensen＇s duct in superflcial parotidectomy can reduce the postoperative complications.
