经上颌窦神经撕脱术治疗三叉神经痛Ⅱ支

Neurectomy of Infraorbital Nerve Through Maxillary Sinus in the Treatment of Trigeminal Neuralgia

目的:评价经上颌窦神经撕脱术治疗三叉神经痛Ⅱ支的疗效。方法:选择1994-08—2009-05期间我科收治的60例经上颌窦神经撕脱术治疗三叉神经痛Ⅱ支的患者进行疗效观察。结果:60例患者手术后均无疼痛,随访1～3年,术后2年疼痛复发6例,术后3年疼痛复发4例,复发率为16.67%。但复发后疼痛均较术前明显减轻,患者能够忍受。所有患者术后除患支区皮肤和黏膜有麻木感外均无其他并发症,麻木感在0.6～1年后逐渐减轻。结论:经上颌窦神经撕脱术治疗三叉神经痛Ⅱ支,效果较好,复发率低,并发症少,是安全有效的治疗方法。

Objective： To summarize the clinical practice of infraorbital nerve avulsion by means of intraoral maxillary sinus incision in treatment of trigeminal neuralgia.Methods： From 1994-08—2009-05,60 patients having trigeminal neuralgia were treated in our department.Retrospective study was taken to see the result of treatment.Results： 60 patients resulted in comlete relief of pain after operation and followed up for 1 to 3 years.6 cases recurred after 2 years,4 cases recurred after 3 years,the recurrence rate was 16.67%.The recurred cases showed incomplete relief of pain but great improvement.All patients experienced the numb feeling associated with this method of treatment and generally got relief for several months.Conclusion： The maxillary sinus nerve avulsion treatment of trigeminal neuralgiaⅡ,is safe and effective.
