中枢神经细胞瘤的MRI诊断价值

The MRI Diagnostic Value of the Central Neurocytoma

目的探讨中枢神经细胞瘤的MRI特点及其诊断价值。方法回顾性分析11例经手术病理证实的中枢神经细胞瘤的MRI表现。结果所有肿瘤均位于侧脑室透明隔,邻近Monro孔,肿瘤形态均不规则;MRI平扫T1WI呈等、低信号,T2WI呈不均匀等、稍高信号。9例肿瘤可见多发囊变,主要位于病灶的外周,肿瘤的周边并可见等信号的索条状结构与侧脑室壁及胼胝体不同程度粘连;4例病灶内可见迂曲的血管流空影;增强检查8例肿瘤实质部分呈中等-明显不均匀强化,4例肿瘤内可见粗大的血管影,囊变周围的索条状结构中等强化,形成较具特征性"丝瓜瓢"状的改变。结论中枢神经细胞瘤MRI表现有一定的特征性,结合年龄特点,有助于与其他脑室内肿瘤进行鉴别。

Objective To investigate the MRI features of the central neurocytoma （CNC） and its diagnostic value. Methods MR imaging of 11 patients with central neurocytoma （CNC） confirmed surgically and pathologically were retrospectively reviewed. Results All the lesions located in the lateral ventricle septum pellucidum, adjacent to Monro hole, with irrugular shape, present as hypointense and equal signal intensity on T1WI and inhomogeneous hyperintense signal on T2WI. 9 lesions were associated with multiple cystic degeneration in the borders of the tumors, as well as there were multiple strip-like structures in the periphery, which adhered to the ventricle walls and callus.Flow void signal intensity were demonstrated in 4 patients. On post-contrast MR scan, the solid part of 8 lesions demonstrated moderate or obvious enhancement, the thick enhanced vessels were found in 4 patients, and the strip-like structures demonstrated moderate enhancement surrounding the cystic part showed 搒ponge gourd？patternlike areas, which was the characteristic appearances. Conclusion Central neurocytoma has the characteristic appearances on MRI,with the specific clinical data, being helpful to the diagnosis and differentiate the neurocytoma from other tumors residing in lateral ventricle.
