磁共振成像对周围型肺癌的诊断价值

The diagnosis value of perihheral pulmonary cancer with magnitic resonance imaging

目的探讨磁共振成像对周围型肺癌的诊断价值。材料与方法回顾分析96例经病理证实的周围型肺癌的磁共振表现,并与其CT表现相对照。结果①对5cm以上肿块,MRI、CT对病灶内空泡、空洞、细支气管征的显示无明显差异(P>0.05);MRI、CT对肿块周围棘突征、血管集束征、短毛刺的显示无明显差异(P>0.05)。②对3-5cm的肿块,MRI、CT对上述征象的显示有明显差异值(P<0.05),CT优于MRI。③对3cm以下结节,MRI、CT对上述征象的显示有显著差异(P<0.01),CT明显优于MRI。④对胸椎、胸膜、胸壁病变侵犯的显示MRI优于CT(P<0.05)。⑤对少量胸腔积液的显示MRI优于CT(P<0.05)。⑥MRI对纵膈肺门淋巴结与变异血管的鉴别优于CT。⑦CT对瘤体周围肺组织情况的显示优于MRI(p<0.05)。⑧CT对钙化的显示优于MRI。结论磁共振成像对周围型肺癌的诊断可提供重要依据,是CT检查的重要补充。

Objective To evaluate the diagnosis value of perihheral pulmonary cancer with magnitic resonance imaging. Material and methods 96 patients of sperihheral pulmonary cancer which were comfirmed by pathology were undergone with magnitic resonance and CT. Results 1.About masses which were above 5 cm, MRI was not significantly superior to CT in significant signs such as lobulation spine,short thorn,vacude,air-bronchogram,vessel convergence（P0.05）. The MRI was no significant difference to CT in the signs of the blood vessel gather bunch, the short thorn （P0.05）. 2.About masses which were 3~5 cm, CT was surperior to MRI in above-mentioned signs（P0.05）. 3.About the masses which were less than 3 cm, CT was obviously surperior to MRI in above-mentioned signs （P0.01）. 4.MRI was surperior to CT in showing the infringed lessons which were in thoracic vertebrae, pleura and chest wall （P0.05）. 5.MRI was surperior to CT in showing mild pleural effussion （P0.05）. 6.MRI was surperior to CT in showing lymphoid in mediastinum and the lhilus pulmonis and discriminating of variation blood vessel. 7.CT was obviously surperior to MRI in showing perihheral pulmonary issues around the masses （p0.05）. 8. CT was obviously surperior to MRI in showing the calcify. Conclusion MRI had the important diagnosis value in diagnosis of the perihheral pulmonary cancer, and can be an the important addition of CT.
