髋关节撞击综合征的多排螺旋CT诊断 被引量:20

摘要 目的探讨髋关节撞击综合征的多排螺旋CT诊断价值。方法选取20例临床诊断髋关节撞击综合征(FAI)患者,所有患者均具有腹股沟区或臀部间断性疼痛病史,休息后疼痛能够得到明显或完全缓解,且髋关节撞击试验阳性,对这些患者行多排螺旋CT平扫及三维重建。结果股骨头颈连接处的前外侧偏移量减低10例;股骨头颈连接处骨性突起5例;股骨头颈连接处皮质下囊性变13例;非圆形股骨头7例;髋臼过深2例;髋臼后倾3例;髋臼边缘软骨下囊变16例;髋臼盂唇的骨化变性11例;关节边缘骨赘形成18例。结论髋关节多排螺旋CT及三维重建检查,可发现股骨头颈连接处及髋臼等解剖异常,对FAI的早期诊断、分型及制定手术治疗方案有重大价值。 Objective To discuss the Diagnostic value of multislice spiral CT of femoro-acetabular impingement(FAI). Methods Twenty FAI patients were enrolled with who CT noncontrast enhanced scan and CT three–dimensional reconstruction were performed to assess the hip joint. Results There are 10 cases of anterolateral offset reduction at the femoral head and neck joint; 5 cases of osseous protuberance at the femoral head and neck joint; 13 cases of subcortical cystic degeneration at the femoral head and neck joint; 7 cases of non-circular femoral head; 2 cases of over-deep acetabulum; 3 cases of acetabular retroversion; 16 cases of subchondral cystic degeneration at the acetabulum margin; 11 cases of acetabular labrum ossification and degeneration;18 cases of osteophyte formation at the joint margin. Conclusion Non-buninoid femoral head, hyper-deep of hip,and typical sings and symptoms lead to early and exactitude diagnosis ,The results of multi spiral CT three-dimensional reconstruction can provid straight reference operation schedule.
