摘要
目的探索艾滋病病毒感染者与人类白细胞抗原(HLA)不同基因型的关系,并评价艾可清胶囊治疗人类免疫缺陷病毒(HIV)感染者的效果。方法收集HIV感染35例,艾滋病(AIDS)患者51例,其中HIV感染者服用艾可清胶囊,AIDS患者服用抗病毒西药,采用聚合酶链式反应-序列特异性引物技术(PCR-SSP)检测所有患者的HLA-B基因型,并进行Karnovsky评分和T淋巴细胞亚群、血常规检测,并对艾可清胶囊疗效的危险因素进行分析。结果 AIDS患者Bw4Bw4基因型的CD4、CD8及Th/Ts治疗后与治疗前相比明显升高,Bw4Bw6基因型的CD4和Th/Ts治疗后与治疗前相比亦明显升高(P<0.01)。HIV感染者不同基因型T细胞治疗前后变化无统计学意义(P>0.05)。艾可清胶囊效佳组治疗后CD4、Karnovsky评分、体重、白细胞、淋巴细胞绝对值均较治疗前明显上升(P<0.05或P<0.01);年龄为影响艾可清胶囊疗效的主要因素。结论携带HLA-Bw4Bw4基因型患者免疫功能恢复快于其他基因型,说明人类自身遗传背景可以影响病情进展,并发现艾可清胶囊可能对40岁以下人群效果更佳。
Objective To explore the correlation between human immunodeficiency virus(HIV) infection patients and human leukocyte antigen genotypes,and to evaluate the therapeutic effect of Aikeqing Capsule(Capsule for AIDS) on HIV infected persons.Methods Thirty-five HIV infected persons and fifty-one AIDS patients were collected.Of them,the HIV infected persons were administered Aikeqing Capsule while AIDS patients medicated with antiviral western medicine.The polymerase chain reaction-sequence specificity primer(PCR-SSP) technique was used to detect the HLA-B genotype of all patients.The Karnovsky score was applied to score the patients.The T lymphocyte subsets and routine blood test were observed.The risk factors of the effect of Aikeqing Capsule were analyzed.Results The CD4,CD8 and Th/Ts of Bw4Bw4 genotype of AIDS patients increased significantly after treatment,and the CD4 and Th/Ts of Bw4Bw6 genotype were also increased significantly(P0.01).The change of T cells of different genotypes of HIV infected persons after treatment was not statistically significant(P0.05).Aikeqing Capsule-effective group had significant increase in CD4,Karnovsky score,body weight,leukocytes,and lymphocyte absolute value in comparing with before treatment(P0.05 or P0.01).Age was the main influential factor of Aikeqing Capsule's effect.Conclusion The immune function recovery of those patients carrying HLA-Bw4Bw4 genotype was faster than that of those carrying other genotypes,indicating that genetic background can influence the progression of disease.Aikeqing Capsule may be more effective for those people under the age of forty.
