17例中老年皮肌炎患者伴发恶性肿瘤的临床分析

Clinical analysis of 17 middle-aged and elderly patients with dermatomyositis associated with malignant tumors

目的研究皮肌炎伴发恶性肿瘤的临床相关因素。方法对近3年来收治的17例皮肌炎合并恶性肿瘤患者的临床资料进行回顾性分析,并将其与45例不合并恶性肿瘤的皮肌炎患者进行比较。结果皮肌炎合并恶性肿瘤发生率为27.4%,肿瘤类型多样以胃癌、卵巢肿瘤、食道癌为主。伴发恶性肿瘤的皮肌炎患者血沉(ESR)、肌酸磷酸激酶(CK)、乳酸脱氢酶(LDH)及-羟丁酸脱氢酶(-HBDH)的水平较不伴发恶性肿瘤者高,肿瘤标志物阳性率较不伴恶性肿瘤患者高。结论年龄>40岁、ESR、CK、LDH、-HBDH及肿瘤标志物等升高为皮肌炎伴发恶性肿瘤的临床相关因素。

Objective To analyze the clinical factors relating dermatomyositis associated with malignant tumors.Methods 17 cases treated in the past 3 years of DM associated with malignant tumors were included in the retrospective analysis,and their clinical information was compared to that of 45 DM patients without malignant tumors.Results The incidence of DM associated with malignant tumors was 27.4%.There were various types of tumors with the leading types of gastric cancer,ovarian cancer and esophageal cancer.The levels of ESR,CK,LDH and α-HBDH of DM patients with malignant tumors were higher than those of DM patients without malignant tumors,and the positive rate of tumor markers was higher too.Conclusions These factors,age of 40 years over,the higher levels of ESR,CK,LDH,and α-HBDH and the high positive rate of tumor markers,are related to DM associated with malignant tumor.
