首发精神分裂症患者额叶白质及海马中脑代谢物质的变化 被引量:3

摘要 目的利用氢质子磁共振波谱(1H-MRS)分析首发精神分裂症患者额叶白质及海马脑代谢物质的变化。方法对符合美国精神障碍诊断与统计手册第4版(DSM-Ⅳ)诊断标准的63例首发未经过药物治疗的精神分裂症患者和63例健康对照,分别测定额叶白质和海马脑代谢产物N-乙酰基天门冬氨酸(NAA)、肌酸(Cr)、胆碱(Cho)、肌醇(MI),以Cr为参照物,分别计算双侧的NAA/Cr、Cho/Cr、MI/Cr以及NAA/CHo,同时用阳性和阴性症状量表(PANSS)对精神分裂症患者进行症状严重程度的评定。结果首发精神分裂症组与正常对照组相比,额叶白质及海马的NAA/Cr、Cho/Cr、MI/Cr及NAA/Cho的组间差异均无统计学意义。首发精神分裂症组PANSS阳性症状评分与右侧额叶白质Cho/Cr存在正相关(r=0.321,P<0.01),与左侧额叶白质NAA/Cho存在负相关(r=-0.367,P<0.01);PANSS阴性症状评分与左侧额叶白质NAA/Cr存在负相关(r=-0.280,P<0.05),与右侧额叶白质NAA/Cr存在负相关(r=-0.372,P<0.01)。结论首发精神分裂症尚未出现明显的额叶白质及海马的损害。额叶白质功能低下可能是首发精神分裂症阳性症状和阴性症状的神经病理基础。
