摘要
当肺泡压升高时,狗右心室压力信号正负微分(RV±dp/dt),右冠脉收缩期、舒张期的灌注压差(LVSP-RVSP、MAP-RAP)均进行性下降。当肺泡压升到2.5kPa时,RV+dp/dt、RV-dp/dt、LVSP-RVSP和MAP-RAP分别下降了62.3%、51.0%、66%和56%。RV+dp/dt与LVSP-RVSP和MAP-RAP的相关性极佳(r=0.8420,r=0.8103,P<0.01),RV-dp/dt与LVSP-RVSP,MAP-RAP的相关性也很好(r=0.8241,r=0.7820,P<0.01)。提示肺泡压升高对冠脉灌注压差,右心室收缩、舒张性能有明显影响,右心室收缩、舒张性能变化与冠脉灌注压差的变化有关。
When alveolar pressure is elevated, the positive and negative differential of the right ventricular pressure signal ( RV±dp/dt ) and right coronary artery systolic and diastolic perfusion pressure difference ( LVSP-RVSP, MAP-RAP ) ave progressively decreased, when alveolar pressure is elevated to 2.5kPa, RV + dp/dt, RV - dp/dt, LVSP-RVSP and MAP-RAP fall 62,3%, 51.0%, 66.0% and- 56.0% respectively. Correlation coefficients of RV + dp/dt and LVSP-RVSP, MAP-RAP ( r = 0,8420 ,and r = 0.8103, P<0.01).are very good; and the correlation coefficients of RV-dp/dt, LVSP-RVSP and MAP-RAP (r = 0.8241,r = 0.7820, P<0.01)also very good, It suggests that elevated alveolar pressure have greater effects on coronary perfusion pressure, right ventricular contractile property and compliance and that the change of right contractile property and compliance be related to that of right coronary perfusion pressure.
