摘要 目的评估CT导引下经皮穿刺活检对胸部病变的诊断价值。方法回顾性研究手术病理或临床追踪证实的CT导引经皮胸部穿刺病变104例。病灶位于上肺野22例,中肺野34例,下肺野38例,纵膈病变10例。病灶大小0.5~10cm,使用18G穿刺针抽吸活检。结果活检成功率100%,穿刺活检诊断为恶性病变67例,良性病变29例、未见恶性征象但无明确良性诊断8例。并发症包括轻度气胸6例,咯血2例,纵隔气肿1例。结论 CT导引下经皮细针胸部穿刺活检是一种安全、准确、有效的辅助诊断方法。 Objective To assess the clinical application of CT-guided percutaneous fine-needle aspiration biopsy of chest lesions. Methods CT-guided percutaneous biopsy was performed with 18G needle in 104 patients that had surgical resection or clinical follow-up. Lesions were located in the upper (n=22),middle (n=34),andlowerlungs (n=38) as well as mediastinum (n=10). The lesions measured from 0.5 cm to 10 cm in diameter. Results CT-guided percntancous aspiration biopsy was performed successfully on all patients with complications ofpneumothorax (6), hemoptysis (2), and mediastinal emphysema (1). Biopsy finding of malignant (67), definitely benign (27), and probably benign (8) lesions were found to be 90% accurate, 92% sensitive and 100% specific. Conclusion CT-guided percutaneous aspiration biopsy is a safe, accurate, and effective method for diagnosis of chest lesions.
