医用臭氧关节内注射治疗膝关节骨性关节炎的临床观察 被引量:15

摘要 目的:观察医用臭氧对膝关节骨性关节炎(knee osteoarthritis,KOA)的治疗作用。方法:采用随机对照设计,将KOA患者96例,随机平均分为观察组和对照组,每组48人。观察组给予浓度30 mg/L的医用臭氧30 mL膝关节内注射;对照组给予玻璃酸钠10 g/L膝关节内注射,两组均每周治疗1次,共治疗4次。治疗前和治疗第4周、治疗第8周、治疗第12周,分别运用视觉模拟评分法(VAS)和Lysholm膝关节评分来进行疗效评定,并对评分结果进行重复测量方差分析。结果:观察组和对照组在治疗后第4周、第8周、第12周时VAS评分均明显低于治疗前(均P<0.05),两组比较在治疗第12周时VAS评分观察组较对照组下降更明显(P<0.05);两组Lysholm膝关节评分在治疗后第4周、第8周、第12周较治疗前有明显提高(均P<0.05),其中治疗后第12周Lysholm膝关节评分,观察组较对照组提高明显(P<0.05)。结论:医用臭氧和玻璃酸钠治疗KOA都能缓解关节疼痛、改善关节功能,医用臭氧的远期疗效优于玻璃酸钠。 Objective:To investigate the clinical effect of medical ozone on treating knee osteoarthritis. Methods :96 cases of knee osteoarthritis were randomly divided into two groups of sodium hyaluronate group( control group), medical ozone group (observing group). The observation group were injected concentration 30 mg/L of medical ozone 30 mL into the knee joint. The control group were injected 10 g/L sodium hyaluronate into the knee joint. Both groups were treated 4 times for all and once per week. Before treatment and 4 weeks, 8 weeks, 12 weeks after treatment respectively the two groups adoped the visual analogue scale (VAS) and the Lysholm knee score to assess clinical effect, and clinical trial results are analyzed with repeated measures analysis of variance. Results:The VAS scores of two groups 4 weeks,8 weeks, 12 weeks after treatment are obviously lower than before treatment, (respectively P 〈 0.05 ), moreover, The VAS score of the observing group decreased more obviously compared with the control group at the 12th week after treatment ( P 〈 0.05 ). The Lysholm knee score of two group 4 weeks ,8 weeks, 12 weeks after treatment obviously increased compared with the score of before treatment ( P 〈 0.05 ). and the Lysholm knee score of the observing group increased more obviously than the control group at the 12th week after treatment(P 〈 0.05 ). Conclusion:Both medical ozone and sodium hyaluronate can relieve joint pain, improve range of motion, but the long curative effects of medical ozone are superior to sodium hyaluronate.
