腹腔镜胆囊切除术3980例临床分析

Laparoscopic Cholecystectomy:Report of 3980 Cases

目的：对６ 年来所开展的 Ｌ Ｃ 进行回顾性分析。方法：１９９１ 年１２ 月－ １９９７ 年１２ 月应用电视腹腔镜治疗良性胆囊疾病３９８０ 例，其中有腹部手术史４２６ 例（１０７ ％ ） ，有全身合并症者３２５ 例（８２ ％ ） 。结果： 治愈３９７９ 例，发生并发症５９ 例，（１５ ％ ） 中转开腹１６ 例（０４０ ％ ） ，死亡１ 例（００２ ％ ） 。结论： Ｉ Ｃ 具有创伤小、痛苦少、恢复快、美容效果好的特点。但对其技术要求高，达到熟练程度难度大，在开始阶段应严格选择病例，不应盲目扩大适应症。手术强调术者技术全面和综合默契配合。

OBJECTIVE:To review the results of laparoscopic cholecystectomy (LC) performed in the past six years.METHODS:There were 3980 laparoscopic cholecystectomies performed for patients with benign glaabladder diseases from December 1991 to December 1997.Among them,426 cases had the history of abdominal operation (10.7%) and 325 cases had other diseases (0.02%).RESULTS:There were 59 complications (1.5%) and one death in this series.16 patients converted to open operations (0.40%).The others all got good recovery.CONCLUSION:LC is a less invasion、less pain、rapid recovery and cosmetiv operation.To master this technique is relatively difficult because it requires the operator had good skills.For a success operation,it is important to select patients strictly especially at the early period of using this technique and good cooperation between each operators is also necessary.
